A new study has identified the recent emergence of a multidrug-resistant strain of Salmonella that has a high level resistance to ciprofloxacin, a common treatment for severe Salmonella infections. The study, led by Fran?ois-Xavier Weill, MD, and Simon Le Hello, PharmD, at the Pasteur Institute in France, is published in The Journal of Infectious Diseases and is now available online.

Salmonella infection represents a major public health problem worldwide. An estimated 1.7 million such infections occur in North America each year. More than 1.6 million cases were reported between 1999 and 2008 in 27 European countries. Although most Salmonella infections produce only mild gastroenteritis, elderly and immunocompromised patients are especially at risk for life-threatening infections. These cases are typically treated with antimicrobials called fluoroquinolones, such as ciprofloxacin.

Dr. Weill and colleagues studied information from national surveillance systems in France, England and Wales, Denmark, and the United States. The data showed that a multidrug-resistant strain of Salmonella, known as S. Kentucky, infected 489 patients in France, England and Wales, and Denmark between 2000 and 2008. In addition, researchers reported that the first infections were acquired mainly in Egypt between 2002 and 2005, while since 2006 the infections have also been acquired in various parts of Africa and the Middle East. The absence of reported international travel in approximately10 percent of the patients suggests that infections may have also occurred in Europe through consumption of contaminated imported foods or through secondary contaminations.

In this study, multidrug-resistant S. Kentucky was isolated from chickens and turkeys from Ethiopia, Morocco, and Nigeria, suggesting that poultry is an important agent for infection. The common use of fluoroquinolones in chicken and turkey production in Nigeria and Morocco may have contributed to this rapid spread.

This study highlights the importance of public health surveillance in a global food system. According to Dr. Le Hello, "We hope that this publication might stir awareness among national and international health, food, and agricultural authorities so that they take the necessary measures to control and stop the dissemination of this strain before it spreads globally, as did another multidrug-resistant strain of Salmonella, Typhimurium DT104, starting in the 1990s." The investigators from the Pasteur Institute and its international network, the Centers for Disease Control and Prevention in the U.S., the Health Protection Agency in the United Kingdom, and the Statens Serum Institute and Technical University of Denmark reported that they will continue to monitor this multidrug-resistant strain as well as help strengthen the capacities of national and regional laboratories in the surveillance of Salmonella and other major foodborne pathogens through the World Health Organization Global Foodborne Infections Network.

Craig Hedberg, PhD, from the University of Minnesota School of Public Health, noted in an accompanying editorial that the ability to integrate public health surveillance is limited by differences in national surveillance systems. The study by Dr. Le Hello and colleagues reported that the percentage of Salmonella isolates submitted from clinical laboratories to national health reference laboratories ranged from 65 percent in France to 99 percent in Denmark. "Given the medical costs and public health impact associated with the spread of multidrug-resistant organisms," Dr. Hedberg noted, "the potential benefits of such a system should far outweigh its costs."

Bay Area cement company given deadline to fix safety violations

State authorities have issued an ultimatum to the oldest cement manufacturer in California, amid growing concern over safety violations.

Lehigh Southwest Cement Co. of Cupertino, which produces half the cement used in the Bay Area each year, has until Aug. 20 to resolve regulatory violations first noted in 2006 or it will be taken off a list of quarries qualified to sell products to government-funded projects, according to the state Office of Mine Reclamation.

The line drawn in the gravel of this community of 58,000, about 45 miles south of San Francisco, divides a politically connected mining operation founded in 1939 from a more recent crop of engineers, computer scientists and environmentalists concerned about pollution emitted from the quarry.

Lehigh officials insist the facilities are safe, in full compliance with state and county regulations, and outfitted with safeguards to reduce emissions of toxic pollutants including mercury. They also warn that the state's action threatens the continued operation of the quarry and the livelihoods of hundreds of people.

Lehigh's proposal to open a second quarry has been bitterly opposed by local residents who fear that it would add pollution to the atmosphere and water, reduce property values and diminish the quality of life.

Yielding to public pressure, the company last year installed a system in its cement plant that reduced mercury emissions by 90%, in part by recycling the heavy metal into cement products. "The mercury levels are extremely low and safe," said Tim Matz, corporate director of environmental affairs for Lehigh. He also noted that "the system has been approved by the U.S. Environmental Protection Agency."

Quarry opponents say the Santa Clara County Board of Supervisors, the lead regulatory agency in the matter, has been hesitant to take action against Lehigh, which employs about 150 people and produces 70% of the cement used in the county each year. Past projects supplied by Lehigh have included the San Jose airport and Bay Bridge reconstruction.

The county had given the company until June 2012 to develop a reclamation plan that would resolve five-year-old violations, including failure to properly stabilize pit slopes, and conducting surface mining and dumping outside of designated boundary lines.

"We were appalled by the failure of the county to enforce the law against this company," said Cupertino resident Richard Adler, a researcher in aging and healthcare. "So we launched an online petition demanding that the state reclamation office finally take Lehigh off the list of mining operations in good standing with the state."

Every time a person added their name to the petition, a computer program designed by the advocacy organization Change.org automatically sent copies of the document to 17 targeted state officials.

An avalanche of petitions overwhelmed state mining authorities.

"I've received more than 23,000 copies of the same petition letter from quarry opponents, so I know they're passionate folks," said Jim Pompy, assistant director of the Office of Mine Reclamation. "But at the same time, Lehigh produces a very important resource for the entire Bay Area, so a lot of careful thought went into the notice we sent the company on July 20."

The quarry is an open-pit operation on a mountain south of Cupertino. Each day, tons of limestone is mined, crushed, blended, melted and heated to produce cement.

Matz, the company's environmental affairs director, said he was "surprised" by the 30-day notice. "It definitely could have an impact on us," he said. "A lot of our projects are government projects."

Last week, a few dozen opponents gathered in Rancho San Antonio in Cupertino to celebrate the state's action.

"Something major has happened and we all deserve credit for it," Adler told the crowd. "We found out about something that was really wrong, we spoke up about it, and got results."

Futures bounce as investors sense debt deal (Reuters)

NEW YORK (Reuters) ? U.S. stock index futures jumped at the start of electronic trading on Sunday evening as investors bet that lawmakers in Washington were set to reach a deal on raising the country's debt limit.

Investors have long believed the debate in Washington would go down to the wire. That appeared to be the case as politicians said they were close to a last-ditch $3 trillion deal to raise the U.S. borrowing limit and avoid a potentially catastrophic default.

"The markets are definitely pricing in a deal," said Peter Kenny, managing director at Knight Capital in Jersey City, New Jersey. "It is clear that an awful lot of movement is going on in Washington and that's what markets are reacting to."

S&P 500 futures rose 16 points and were above fair value, a formula that evaluates pricing by taking into account interest rates, dividends and time to expiration on the contract. Dow Jones industrial average futures rose 125 points, and Nasdaq 100 futures gained 31.75 points.

With two days remaining until the United States exhausts its ability to borrow money, legislators said a deal was becoming more likely, but they had some way to go before reaching agreement.

Wall Street closed its worst week in a year last week after five straight days of losses. Analysts said the oversold conditions had primed the market for a bounce.

"With the percentage of stocks over their own 10-day moving average at an oversold 12.9 percent, a short-term rally may be on the cards based on the arrival of some good news," said Wayne Kaufman, chief market analyst at John Thomas Financial in New York.

"Our fear is that the issue could drag on longer than anyone thinks."

The U.S. dollar received an early lift against the yen and Swiss franc in Asia-Pacific trade on Monday amid the reports that a deal on the U.S. debt wrangle was close.

Any relief rally could be short-lived, investors said, with other big risks present. A report on Friday showed U.S. economic growth in the first half was much slower than anticipated.

The United States could also face a downgrade to its gold-plated AAA sovereign debt rating, which could weigh on markets if that happens later in the year.

"Even if the debt ceiling is raised, all of the heavy lifting will be in front of us," said Peter Kenny, managing director in institutional sales at Knight Capital Group in Jersey City, New Jersey.

"I think it's an almost foregone conclusion that there is going to be a downgrade at some point."

In recent months, we?ve been learning about real estate impresario Dwight Whiting, whose El Toro brainchild became today?s city of Lake Forest. Last week, we met his in-laws, the highly personable Judge and Mrs. Keating. Lest you think we?ve forgotten the woman who linked them?Mrs. Dwight Whiting, the former Emily Sutherland Keating?well, here she is.

Although her portrait has faded with time, it still gives us a good idea why a sophisticated world traveler and land entrepreneur like Whiting fell immediately in love. As family lore has it, moments after Dwight first saw Emily at the Hotel del Coronado?s February 1888 opening, he walked over to the state-of-the-art?elevator she had just boarded and presented her with a bunch of violets hastily purchased from a nearby vendor. Within the week, they were engaged.

SAN DIEGO SOCIETY

As we learned last week, Emily?s older brother George James Keating was the impetus behind the move she and her parents made from Nova Scotia to California. George had made his fortune?more than $2 million?through the manufacture of agricultural implements.? About two years into his second marriage, George decided to move to San Diego for health reasons. But health did not preclude an active social life. George and his wife, Fannie, quickly became part of San Diego?s social scene, entertaining lavishly in their beautiful mansion, The Dells.

Very likely, Dwight Whiting knew something about the San Diego Keatings when he attended the Hotel del Coronado?s first-day ceremonies. Like George Keating, Whiting was ambitious, high-energy and had chosen California for health reasons, although his were related to asthma whereas George had a history of heart trouble.

So, it is not difficult to understand why Emily would have been attracted to the somewhat older (by about eight years) Whiting. In addition to his burly, bearded good looks, Dwight?like her brother George?also had the aura of success about him.

COUNTRY WIFE VS. CITY SOCIALITE

In El Toro, according to historian Clara Mason Fox, Dwight and his wife, along with their newborn daughter, Nathalie, lived for a time at the hotel not far from the train depot. But this was only until their new home had been built. Not long after, according to El Toro historian Joe Osterman, ?a grand home on a small hill? was completed for Dwight?s in-laws, Judge and Mrs. Keating.

All would seem fine, and in many ways it probably was. Dwight?s in-laws were delighted with life in El Toro; he was busy coming up with ideas to develop the little hamlet and, in July 1891, Emily gave birth to a son, Dwight Anson Whiting. Four years later, in June 1895, they had a second son, George.

But it seems that Emily, unlike her parents, did not take to rural life, and the health of her daughter was a concern. Accounts vary as to how old?Nathalie?was when she passed away, but by 1900, Emily came to regard the town?s relative isolation and loss of her oldest child as, in current vernacular, ?deal-breakers.?

Fortunately by this time, Dwight Whiting had agreed to run his ?ranche? from a distance. After he and Emily moved to a Victorian-style mansion in Los Angeles, she began immersing herself in theater-going and other activities that were unavailable in fledgling El Toro.

Although petite?family lore indicates she was no taller than 5-foot-2?and innately feminine, Emily become known as the first Los Angeles resident to drive an electric car. She and Dwight were known to travel up and down the California coastline for both business and pleasure. In 1906, it was reported that ?Mrs. Dwight Whiting of Los Angeles has let the contract for a handsome two-story cottage on First Street? in Bayside, an area under development by Phillip A. Stanton that would later become Seal Beach.

TROUBLE AHEAD

On March 19, 1907, Emily wrote to another Saddleback Valley land baron, Louis Moulton, who had been assisting the Whitings in handling various El Toro business matters.

She and Dwight had been in the area, she wrote, and had called upon him but ?were disappointed that you were not at home.? With the note, Emily added, she was enclosing a check for $190 to cover a mortgage payment on St. George?s Mission. ?Both my husband and I want very much to have the pleasure of your company at dinner some night when you are in town and [we] can put you up for the night,? she said, requesting that he telephone first because ?we do not want to miss you . . . on Saturday and Sunday next we shall be in Santa Barbara!? Closing, she once again mentioned ?Dwight and I are hoping soon to see you.?

Whether Moulton did manage to visit the Whitings at their Los Angeles address is not known. But online records indicate that less than two months later, on May 1, 1907, Dwight passed away at age 53.

Correspondence from the widowed Emily to Louis Moulton that August indicates a woman overwhelmed by financial details. By September, her oldest son, Dwight Anson, who had just turned 16, penned a brief note to Moulton stating, ?Mother asked me to write to you and ask you to please return some letters of hers which you promised to do, but I suppose you have forgotten about it. She is coming down on Tuesday [and] about one o?clock she will be ready to take the drive with you to see the country road, etc.?? (This last reference was to a road through the Whiting estate that had been petitioned for public right-of-way.)

In a letter penned the following day, Emily states her concern for the eucalyptus planted and nurtured by her late husband, adding, ?I do not think you a good judge of what is best for Forest welfare for you do not value it (commercially). I do?almost beyond the ranch itself.?

The outcome of these matters is?at this point in time, for this columnist?shrouded in mystery.

LIFE AFTER DWIGHT

What became of Emily after the initial shock of becoming a widow and shouldering so many concerns? We know she?saw both of her sons through college and marriage and eventually became a grandmother. We also know Emily delved into matrimony again, not once but twice.

Of Emily?s second husband, the most I have found is that his name was Eyre Barrow Finch, born the same year as Emily (1863), and that he was buried?at?El Toro?s cemetery in 1910, making her a widow once more.

Her third marriage, to Gregory Perkins Jr., showed definite signs of distress: By 1915, Emily petitioned for divorce on grounds of extreme mental cruelty.

Emily passed away Nov. 8, 1933, at age 70, around the time her sons were rehabilitating the Serrano adobe (the centerpiece of today?s Heritage Hill Historical Park) into a vacation home and hunting lodge.
